Medical Benefits
We understand that every employee’s needs are different. This is why we offer multiple medical plans to accommodate you and your family. Depending on the medical plan you choose, you may pay an annual deductible before the plan kicks in. You would be responsible for any copays, coinsurance and other costs related to services utilized under the plan.
The United Healthcare plans are available nationwide, and if you reside in Washington, Oregon or California, you may alternatively choose a Kaiser Permanente plan.

2025 Medical Plans
Healthcare Choice (EPO) Plan
Administered by: United Healthcare
The Healthcare Choice Plan provides complete medical services from a preferred network of doctors, hospitals and other care providers. Generally, only services administered by in-network providers will be covered. With no deductible and low, fixed copayments, this plan is ideal for many.
Healthcare Choice Plus (PPO 500) Plan
Administered by: United Healthcare
The Healthcare Choice Plus Plan provides complete medical services from a preferred network of doctors, hospitals and other care providers. This Healthcare Choice Plus plan allows you to receive care from out-of-network providers with a percentage of the costs being covered by the Plan.
HSA Choice Plus (PPO 2250) Plan
Administered by: United Healthcare
The HSA Choice Plus Plan is similar to the Healthcare Choice Plus plan, but with a much higher out-of-pocket deductible ($2,250 individual/$4,500 family). This plan qualifies you to contribute pre-tax dollars to a Health Savings Account (HSA) and receive the company's matching contribution.
HMO and HSA Plans
Administered by: Kaiser Permanente
An HMO Plan and an HSA-Qualified Deductible HMO Plan are available if you reside in California, Washington or Oregon. They provide complete medical services from an exclusive network of Kaiser providers. As a Kaiser member, you are required to stay within the Kaiser network for all non-emergency care.
